People Connect Carepath
When a health crisis happens, employees need end to end support. Through People Connect, Carepath provides individualized nurse case management for all types of medical conditions providing navigation from diagnosis, treatment & ongoing care. A single point of contact with one of our Nurse Case Managers throughout treatment maintains continuity of care and ensures employees and their families receive the right treatment, at the right time, and at the right place.
Core elements of the program include
Health Care Navigation
Helping employees navigate the health care system, improving both the quality of care and emotional support.
Providing medical second opinions through “the Clinic” powered by Cleveland Clinic/American Well.
Our cancer program assigns an oncology nurse specialists to provide ongoing support, information, guidance and expertise.
Mental Health Care
Emotional support and virtual therapy, assessments and psychiatrist collaboration through People Connect.

Carepath leverages telephonic and virtual visits which allow timely, safe and efficient care for patients and their families.
The following services are provided by Carepath under People Connect:
- Assistance in accessing appropriate medical resources
- Conduct doctor-to-doctor consults with treating physicians
- Coordinate specialist referrals with family doctors
- Facilitate access to tests, treatments and clinical trials
- Refer to mental health assessments and support
- Engage with therapist and psychiatrist for a collaborative approach
- Access physiotherapy, occupational therapy, speech language pathology, dietitian services, pain and palliative care programs
Virtual Health Care Navigation
Carepath’s Health Care Navigation program provides individualized case management for all medical conditions. A single point of contact with one Nurse Case Manager maintains continuity in care.
All services are delivered virtually via telephone or through secure video conferencing facilitating timely and efficient support. Your Nurse Case Manager is here to assist you through diagnosis, treatment and plan of care. Our program works in parallel to the public health care system and does not replace your current health care team.
How our program works:
When you call our toll-free number you will be connected with our intake team who will ask you some general questions. An email is then sent with a link to our program consent forms.
Upon completion of the documents, a Nurse Case Manager will contact you within 1-2 business days. This is an opportunity for our nursing team to answer any questions regarding our navigation program, conduct a thorough review of your medical history, current diagnosis, associated symptoms, and to discuss any other questions you may have about your condition.
Services Include
- Scheduled telephone appointments based on your agenda and support needs;
- Thorough review of your treatment plan ensuring consistency with evidence-based practice;
- Ongoing health education and coaching;
- Explanation of options for diagnostic testing and treatment(s);
- Assistance with how to access diagnostic tests and clinical trials;
- In partnership with the Clinic powered by Cleveland Clinic/American Well, you are eligible for a medical second opinion when requested;
- Navigation through local health care resources and treatment locations;
- Review of private care options;
- Development of questions for utilization during medical appointments;
- Telephonic or secure video conferencing medical and emotional support
- Navigation through community organizations;
- Assistance in how to access home care, physiotherapy, occupational therapy, speech language pathology, dietitian services, pain programs, social workers, and many other community care resources;
- Empowerment to take an active role in your treatment and recovery plan;
- Support in making informed health care decisions.
Upon completion of the navigation your Nurse Case Manager will provide a comprehensive summary of the resources provided during your journey.
An optional Carepath Satisfaction Survey used to improve the delivery and experience of our services will also be sent out.
